How to request deletion of your data

Website data

This website keeps no personal information other than short-lived server logs held by our hosting provider. If you believe we hold website data about you, email operations@itforfire.com with the subject "Data deletion request" and the email address you used. We confirm receipt, delete what we can, and reply within 45 days.

Patient records, portal account, appointments and messages

These are held by the practice in SimplePractice, not by this website. Call (510) 473-8008 or write to Alameda Disc & Spine, 1150 Ballena Blvd, Suite 220, Alameda, CA 94501. Please note that California law requires us to retain medical records for set periods, so clinical records cannot be deleted on request, though you can request copies and corrections at any time. Your SimplePractice Client Portal account can be closed on request.
